While it really depends on the location, opponent, and the type of grab, I will usually do something similar to what Tim described.
I grab my opponent's neck with the hand that is farthest away from his elbow and pull the elbow that is wrapped around my leg with the hand/arm that is closest to him as soon as I have the gripped his neck and his elbow I will jump into them as I kick my leg towards the ground.
As I jump into him I move my hand that is grabbing around his neck to the other side, on the same side as my trapped leg, and push his head down and away as I continue to move my leg down and my hips away until I break their grip on my leg.
